Structured Alert & Escalation Model

Incoming measurements are automatically evaluated against predefined clinical thresholds. Deviations trigger categorized alerts (critical, high, medium, low), clearly highlighted within the monitoring dashboard.
 

When a critical alert occurs:
 

  1. The assigned nurse reviews the case

  2. The measurement is validated and contextualized

  3. The resident may be contacted directly

  4. The case is escalated to the treating physician if required
     

Escalation protocols are predefined and aligned with clinical governance standards, ensuring consistency, safety, and accountability.

From Alert to Clinical Action

Upon escalation, the treating physician determines the appropriate course of action, which may include:
 

  • Adjustment of medication or therapy

  • Increased monitoring frequency

  • Activation of hospital-at-home interventions

  • Hospital admission if clinically necessary
